Poor drees does not disqualify you. It makes an extra hurdle you need to jump before you get a chance to qualify. You can still be successful when you are not dressed well.
Unfortunately, people judge you based on how you look. Women, according to MSN.com. judge an approaching man based on his shoes, hair, and accented items such as jewelry and wristbands. They assess whether you are worth investing time into talking to.
Looking the part means building your avatar. Building an avatar is not normally a all-of-a-sudden thing. It's evolving. You start with a couple changes that transition you into what you want to be. You add to the look as you find items that represent that avatar. You remove items that no longer associate with the avatar.

No. Getting back out there is the first thing you should be doing. Learn and develop as you continue to go out there. You will find the motivation to do the other things will come from a desire to be more successful in the field.

If the only wing you can find is a dlv, go solo. Sure, you have to then establish acceptance, but you do that by opening the biggest set in the room, solely for the goal of interacting, not getting a target. Once you get them, move on and people will see you were accepted by the first group.
